Mounted on the top of the plaque is a black metal rectangular metal panel with a thing gold metal border that has a metal floral decal at each corner. A gold-tone, circular, relief medallion featuring an image of three faces - one forward facing and two side profiles is adhered to the center top of the plaque. At the back of the plaque, are mounting holes at the top and proper right.
