Across the top margin is plain black text that reads “INFORMATION FOR PERSONS BEING DISCHARGED OR RELEASED / FROM ACTIVE SERVICE.” The sheet lists organizations that are authorized to assist veterans. There is a handwritten check mark in black ink next to the name for the American Legion. At the bottom of the sheet are two blank spaces with stamps in blue ink. The first is a blank space for the veteran having filed a claim for disability benefits. The stamp reads “FEB 28, 1946 / [DATE OF FILING].” the second stamp reads "CAMP BEALE." Along the bottom margin is black text that reads “KEEP THIS WITH YOUR DISCHARGE CERTIFICATE.” This back of the sheet is blank.
